Please bring your valid, unexpired health insurance card (copy not allowed)
to your first visit, or to your visit at the beginning of the month.

If your health insurance changes due to employment or change of job, please
present your new insurance card at the reception.

Please let the receptionist know if there are any changes to your insurance
information, such as your address or phone number.

If you have a beneficiary certificate (medical care for the elderly,
disabled, etc.), please bring it with your health insurance card.

After returning to the clinic, please bring your "medical examination
ticket" (診察券) with you each time you visit the clinic.

If you are prescribed medicine at another medical institution, please show
your medication list.

If you are pregnant or could be pregnant, please be sure to let us know.

If you have severe pain, high fever, vomiting, etc., please let the
receptionist know

To ensure your visit proceeds smoothly, please tell us about your current
medical history (when and what kind of symptoms appeared), your medical
history (illnesses you have had), and your health (examination) examination
results.

Those up to junior high school students should be accompanied by a guardian.
Those under the age of 18 who wish to receive a vaccination must also be
accompanied by a guardian.
